Surgery. a. The Allowable Charge for Inpatient and Outpatient Surgery includes all pre-operative and post-operative medical visits. b. The pre-operative and post-operative period is defined and determined by Us and is that period of time which is appropriate as routine care for the particular surgical procedure. c. When performed in the Physician's office, the Allowable Charge for the Surgery includes the office visit. No additional Benefits are allowed toward charges for office visits on the same day as the Surgery.
