Terminally Ill. Shall be defined as a medical prognosis of limited expected survival of six (6) months or less at the time of referral to a Hospice of a Covered Person with a chronic, progressive Illness which has been designated by the Covered Person's attending Physician as incurable.

Terminally Ill. The Carrier will provide a waiver for Life Insurance only and not EHC or Dental. Therefore as a statement in the Memo: In the face of a terminally ill employee – letter to the effect that the parties will meet to determine methods of cost sharing the cost of continuing extended health and dental benefits coverage for a further period not to exceed six (6) months.
