Tobacco Dependence Treatment Services. The Contractor shall cover, at minimum, Tobacco Dependence Treatment services as set forth in 405 IAC 5-37. Treatment shall include prescription of any combination of tobacco dependence products and counseling. Providers may prescribe one or more modalities of treatment. Providers shall include counseling in any combination of treatment. Pharmacotherapy treatment for Hoosier Healthwise members shall be included as well.
