The Role of the Loddon Children’s Health and Wellbeing Local Team The Loddon Children’s Health and Wellbeing Local (LCHWL) provides a range of health and wellbeing services to children up to 11 years of age who may be experiencing some challenges such as developmental, emotional, relational or behavioural concerns. Services are available to families across the Xxxxxx of Campaspe, Central Goldfields, Loddon, Macedon Ranges and Mt Alexander, and City of Greater Bendigo. The LCHWL is an innovative model co-designed with partners – paediatric service providers, and people with lived experience - to provide a regionally based system response that improves access across prevention, primary and tertiary care in the Loddon region. The service is particularly targeting those children who are not accessing support under other mechanisms such as NDIS. Services provided through the LCHWL include paediatric health, mental health support, parenting support, specialist assessments, allied health, family services, care coordination and access to peer support from team members with a lived or living experience of caring for a child with developmental, emotional, relational or behavioural concerns. The LCHWL has recently been established in response to the Royal Commission into Victoria’s Mental Health System and led by Bendigo Community Health Services (BCHS) in partnership with Bendigo Health and regional partners Bendigo and District Aboriginal Cooperative, Njernda Aboriginal Corporation, Echuca Regional Health, Dhelkaya Health, Sunbury & Cobaw Community Health, North Central LLEN and Maryborough District Health. Position Summary This position is employed by BCHS and works as an integral part of the LCHWL team. Lived and living experience roles within the LCHWL will bring their experience of parenting a child with developmental, social, emotional or behavioural concerns to support others and help them navigate their current family goals and challenges. Purposefully sharing stories and reflections with people who have had a similar experience provides a valuable support to people, assisting information sharing, learning and offering hope and assistance in a safe and welcoming environment. In addition to engaging with families accessing the LCHWL, the position will also offer support to other lived experience roles within the LCHWL, contributing to a well-supported team of peer workers working across the service. The position will focus on engaging with families from their first point of contact with the LCHWL, and/or as requested across the family’s engagement with supports, establishing connection with parents/carers in person or via phone or video call. The position will offer targeted follow up with parents/carers, bringing a peer understanding to the relationship. The position will also work as part of the multiservice team, supporting the development of programs that are family centred through the contribution of lived experience knowledge. The Role of the Loddon Children’s Health Team Sexual Assault Multidisciplinary Centres (MDCs) were introduced in 2006 to improve responses to sexual offences and Wellbeing Local Team The Loddon Children’s Health and Wellbeing Local (LCHWL) provides a range of health and wellbeing services to children up to 11 years of age who may be experiencing some challenges such as developmental, emotional, relational or behavioural concerns. Services are available to families across the Xxxxxx of Campaspe, Central Goldfields, Loddon, Macedon Ranges and Mt Alexander, and City of Greater Bendigochild sexual abuse. The LCHWL is an innovative model centres co-designed locate child protection practitioners, specialist police investigators and sexual assault counsellor/advocates with partners – paediatric strong links to forensic medical personnel. These professionals work collaboratively to provide a victim/survivor centred, specialist, integrated and holistic response to victims of sexual abuse from a single location. This ensures that victim/survivors experience fewer service providers, more timely and people accessible responses, increased sensitivity, privacy, and anonymity. The state government expanded the MDC model to incorporate a community health nurse at each MDC. The community health nurse will have a particular scope and focus, noting the importance of integration and linkages with lived experience - the rest of the health system and appropriate structures for support and governance. The MDC community health nurses will practice within the social model of health, providing needs identification, care planning, referral to appropriate services providers, education awareness- raising and develop local networks of care. Community health nurses will provide a regionally based system response that improves access across prevention, primary short to medium term care and tertiary care provide an important link with other health and community services. They will also partner with other providers to assist with quality improvement and service planning activities for survivors of sexual assault. Community health nurses will report operationally to their employing community health centres whilst they primarily work in the Loddon regionsix Multi-disciplinary centres across Victoria, however, they will also have access to a state-wide Coordinator who will provide governance and leadership support. The service is particularly targeting those children who are not accessing support under other mechanisms such as NDIS. The LCHWL has recently been established in response to the Royal Commission into Victoria’s Mental Health System and led by Bendigo Community Health Services (BCHS) in partnership with Bendigo Health and regional partners Bendigo and District Aboriginal Cooperative, Njernda Aboriginal Corporation, Echuca Regional Health, Dhelkaya Health, Sunbury & Cobaw Community Health, North Central LLEN and Maryborough District Health. Position Summary This position is employed by BCHS and works as an integral part of the Better Health portfolio that also involves the planning and provision of medical, paediatrics, chronic disease management, sexual and reproductive Health, refugee health nursing, workplace health, multi-disciplinary centre nurse, social support in Xxxxxx, physiotherapy, dietetics and GP’s in Schools. Position Role This exciting new role gives a passionate clinician the opportunity to be a part of the innovative LCHWL team. Lived delivering holistic care, ensuring coordinated responses to the changing needs of children and living experience roles within the LCHWL will bring their experience of parenting a child families and working collaboratively with developmental, social, emotional or behavioural concerns to support others and help them navigate their current family goals and challenges. Purposefully sharing stories and reflections with people who have had a similar experience provides a valuable support to people, assisting information sharing, learning and offering hope and assistance in a safe and welcoming environment. In addition to engaging with families accessing the LCHWL, the position will also offer support to other lived experience roles within the LCHWL, contributing to a well-supported team of peer workers working across the servicepartner organisations. The position will focus on engaging provide an opportunity for a suitably qualified and experienced Paediatric Nurse to work with children and their families from their first point of contact with the LCHWL, and/or as requested across the family’s engagement with supports, establishing connection with parents/carers in person or via phone or video callLoddon catchment to assist the child and their families to reach their full potential using a multidisciplinary approach and case coordination. The position Paediatric Nurse will offer targeted follow up work in collaboration with parents/carersa team including Paediatricians, bringing GPs, Nurses, Speech Pathologists, Psychiatrist, Family Services and Mental Health clinicians to provide a peer understanding to the relationship. The position will also work as part of the multiservice team, supporting the development of programs that are family centred through the contribution of lived experience knowledgeholistic assessment and management ensuring early intervention for children and their families. Position Responsibilities The responsibilities of the position are: • Provide a welcoming environment Work collaboratively to provide evidence-based assessment and engagement opportunity to people accessing the LCHWL through sharedinterventions of children experiencing developmental, lived experienceemotional, relational, and behavioural challenges. • Work Gather information and develop care plans to minimise families having to repeatedly tell their story. • Support tailored and coordinated care with therapeutic coherence. • Advocate for service based on identified needs of children and families and contribute to an evolving model of care. • Provide intervention/s for patients, including the provision of a range of evidence-based approaches such as: brief interventions; single session therapy; and other strategies suitable for use with individuals experiencing psychological distress. • Undertake facilitated referrals, warm referrals or connections/linkages to other services. • Participate in internal case coordination and conferencing as a member of the LCHWL a multi- disciplinary team to support child and family access to LCHWL services and broader service system navigationacross partner organisations. • Establish connections with families in individual and/or group settings as they engage in LCHWL services. • Facilitate one to one peer sessions with parents/carers. • Share Accurately record and discuss common experiences with parents/carers to build trust document client and rapport. • Take a strength-based approach to empower and validate parent/carer experience, being led by the parent/carer’s choices. • Offer targeted support to parents/carers, which will include speaking with families new to the service, offering support to families who may be waiting for service, and connecting with families as they transition out of the service. • Provide administrative information and resources to parents/carers. • Align all engagement with local families with the LCHWL client journey policies collect and procedures. • Contribute to a positive experience for children and families through the provision of family led care planning. • Support the LCHWL lived experience team through mentoring and day to day support. • Actively engage in the multi-disciplinary team to share information and knowledge in support of the best outcomes for children and familiescollate data within given timeframes. • Participate in all relevant LCHWL service meetingscontinuous quality improvement activities. • Complete data collection and client case notes Participate in the established information management systemsdevelopment of policies, procedures and processes that impact on the service and on the achievement of a high-quality service provision and relevant accreditation standards. • Participation Maintain and develop professional skills, in supervision tailored addition to monitoring new developments through continuing professional development. • Participate in the peer support rolemonitoring and review of the demand for LCHWL services to ensure services remain accessible and equitable. • Other duties as directed..
