Transition/Exit Planning. CONTRACTOR shall begin discharge planning 20 immediately after enrollment. CONTRACTOR shall develop a formal exit plan with the Client no later 21 than fourteen (14) calendar days prior to Client’s successful completion of the program. The exit plan 22 shall be completed and signed by CONTRACTOR staff and Client. The exit plan shall be documented 23 in the Client’s chart and shall: 24 1) Include a plan to assist the Client in maintaining a continued alcohol and drug f ree 25 lifestyle; 26 2) Address goals on the treatment plan; and 27 3) Include referrals to appropriate resources such as outpatient treatment, self-help 28 groups, alumni groups, recovery maintenance services, social services, vocational rehabilitation, job 29 training, and /or other services as needed.
