Vaccination Certificates and Medical Reports. Successful Service Provider(s) will be required to provide proof of applicable valid vaccination certificates or medical reports from a registered Medical Practitioner (Doctor) prior to the commencement of the contract. This is a preventative measure to ensure the health and safety of all field employees working with sewage.
