Medical Reports. The Employer agrees to pay the fee for medical reports required by the Employer for Sick Leave or Weekly Indemnity provisions to a maximum of fifty dollars ($50.00).

Medical Reports. The following applies to all employees:
Medical Reports. The Employer agrees to pay the full cost for the report required by the Employer for Weekly Indemnity benefits to a maximum of fifty ($50.00) dollars. The employee shall submit a receipt to the Employer. The Employer will pay the reimbursement described above to a maximum of three (3) reports related to a specific claim.
Medical Reports. Teachers returning from a medical leave shall submit the appropriate form from their attending physician indicating that they have medical clearance to return prior to reporting to work. Medical Reports. The Employer shall, at its request, be provided with a written medical verification of an employee's illness for sick leave. If a cost is incurred by the employee, the Employer shall pay up to twenty dollars ($20.00) toward same. Where the Employer requests a detailed medical report to verify an illness for sick leave, the Employer shall pay up to forty dollars (40.00) for same.
Medical Reports. Subject to provision (4.1), the Insurer has the right and the claimant shall afford to a duly qualified medical practitioner named by the Insurer an opportunity to examine the person of the insured’s person when and as often as it reasonably requires while the claim is pending, and also, in the case of the death of the insured person, to make an autopsy subject to the law relating to autopsies.
