Common use of Visitors Clause in Contracts

Visitors. Family members* temporarily visiting a Patient from out of town may, for a two- week period, take advantage of the services described in subparagraphs (a), (c), and (d) of this paragraph. Medical services rendered to Patient’s visitors shall be charged on a fee-for-service basis. *Family members who are Medicare beneficiaries must be covered by a Medicare opt out and waiver agreement in order to be treated by a Mind & Body Christian Physician.

Visitors. Family members* members temporarily visiting a Patient from out of town may, for a two- week period, take advantage of the services described in subparagraphs (a), (c), and (d) of this paragraph. Medical services rendered to Patient’s visitors shall be charged on a fee-for-service basis. *Family members who are Medicare beneficiaries must be covered by a Medicare opt out and waiver agreement in order to be treated by a Mind & Body Christian New South Family Medicine, LLC Physician.

Medical Patient services rendered to Patient’s visitors shall be charged on a fee-for-service basis. *Family members who are Medicare Medicare, Medicaid, or other Federal Health Plan beneficiaries must be covered are currently NOT ELEGIBLE for treatment by a Medicare opt out and waiver agreement in order to be treated by a Mind & Body Christian PhysicianHEALTH FROM HOME Provider.

Medical services rendered to Patient’s 's visitors shall be charged on a fee-for-service for- service- performed basis, subject to the terms and conditions of the Agreement between Patient and Functional Family. *Family members who are Medicare beneficiaries must be covered by a Medicare opt Opt-out and waiver agreement & Waiver Agreement in order to be treated by a Mind & Body Christian PhysicianFunctional Family.

Family members* Non-Medicare family members temporarily visiting a Patient from out of town may, for a two- two-week period, take advantage of the services described in subparagraphs (a), (c), and (d) of this paragraph. Medical services rendered to Patient’s visitors shall be charged on a fee-for-service basis. *Family members who are Medicare beneficiaries must be covered by a Medicare opt out After the 2 week period, any further services and waiver agreement in order to be treated by a Mind & Body Christian Physicianpayment is at the discretion of the physician.
