WAIVER OF CLAIMS AGREEMENT. In consideration of allowing me to participate in the League’s sport‐play activities at the Complex, I hereby, for myself and if applicable, for my minor child, to the fullest extent permitted by law:

WAIVER OF CLAIMS AGREEMENT. For allowing me to participate in the MFSC ice times, special show/events,public skating and/or City Ice Times without a helmet at the Mentor Ice Arena, I agree, to the fullest extent permitted by law as follows:
WAIVER OF CLAIMS AGREEMENT. In consideration of allowing the Club to participate in any League sanctioned and/or unsanctioned ice skating activities at the Arena, I hereby agree as follows:
WAIVER OF CLAIMS AGREEMENT. In consideration of allowing me to participate in ice skating, soccer, lacrosse, and/or turf sports activities at the Arena, I hereby agree as follows: TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have against the Arena, resulting from activities at the Arena. TO RELEASE the Arena from any and all liability for any loss, damage, injury or expense that I may suffer, or that my next of kin may suffer, as a result of my participation in the activity described in this Agreement, due to any cause whatsoever, including
