Common use of Withdrawal Application and Conditions Clause in Contracts

Withdrawal Application and Conditions. In order to withdraw days from the Sick Leave Bank, the member shall submit a written application to the Superintendent accompanied by written certification from the attending medical doctor indicating the extent of the illness or injury and the anticipated length of the required absence from work. The request for the withdrawal of days from the Bank shall be denied if the member fails to substantiate properly the medical necessity for the leave period. The member withdrawing days from the Bank shall not be required to repay the Bank for the days withdrawn except as a regular contributing member of the Bank as provided in Section 1004.2 above.
