You or Your authorized representative must write to Us within 60 calendar days after Our decision for the first-level Appeal. If We receive Your request after 60 days, We will not consider it. Write to: Blue Cross and Blue Shield of Louisiana Appeals and Grievance Unit P. O. Box 98045 Baton Rouge, LA 70898-9045 A Member Appeals Committee whose members were not involved in Your case before will meet to review Your request. The meeting is normally held within 45 working days after We receive a request for a second-level Appeal. Once the Committee decides on Your case, it will mail its decision to You within 5 working days after the meeting. The Committee’s decision is final and binding.
