THIS DEALER A is made this day of , 20 (“Effective Date”) by and between American Guardian Warranty Services, Inc., an Illinois corporation, American Guardian Warranty Services of Florida, Inc., a Florida corporation, American Guardian Warranty Services of Wisconsin, Inc., an Illinois corporation (hereinafter individually or collectively “AGWS”) and the undersigned entity outlined below and on the Dealer Information Sheet attached hereto (“Dealer”).
