This agreement for the provision of sports medicine services (“Agreement”), entered into as of August 1, 2020 (“Effective Date”), is by and between Greater Clark County School Board of Trustees, on behalf of Charlestown High School, Charlestown Middle School, Jeffersonville High School, and New Washington High School (hereinafter collectively referred to as “School”) and Baptist Health Medical Group, Inc. d/b/a Baptist Sports Medicine (hereinafter “Baptist”) and governs the provision of sports medicine services to School.
