This agreement for the provision of sports medicine services (“Agreement”), entered into as of August 1, 2020 (“Effective Date”), is by and between Greater Clark County School Board of Trustees, on behalf of Charlestown High School, Charlestown Middle School, Jeffersonville High School, and New Washington High School (hereinafter collectively referred to as “School”) and Baptist Health Medical Group, Inc. d/b/a Baptist Sports Medicine (hereinafter “Baptist”) and governs the provision of sports medicine services to School.

SportsPlus Athletic Training is the owner and operator of an athletic training business which operates a sports medicine department. Las Positas College conducts an athletic program for its students (the “Program”). The Program wishes to retain SportsPlus Athletic Training to provide sports medicine services for the Program, and SportsPlus Athletic Training wishes to provide sports medicine services for the Program on the following terms:

This Sports Medicine Services Agreement is entered into between School and/or Board on the terms indicated below and those set forth on the Sports Medicine Services Agreement Terms and Conditions (the “Terms”), which are attached hereto and incorporated herein by this reference. The parties acknowledge and agree that this Sports Medicine Services Agreement replaces and supersedes that certain prior Services Agreement between Richmond Heights High School and Hospital, effective July 1, 2012. In recognition of the Acknowledgements set forth in the Terms, the parties, intending to be bound, hereby agree that School engages Hospital as set forth and provided for below:
