CLINICAL STUDY AGREEMENT SMLOUVA O KLINICKÉ STUDII This Clinical Study Agreement (“Agreement”) is made and entered into as of 20-Dec-2016 (“Effective Date”) by and between, Ústav Hematologie a krevní transfuze located at U Nemocnice 2094/1, 128 20 Praha 2, Czech Republic (“Study Site”), located at U Nemocnice 2094/1, 128 20 Praha 2, Czech Republic (“Principal Investigator”), and Daiichi Sankyo, Inc., located at 399 Thornall Street, Edison, New Jersey 08837, USA (“DSI”).
