I, (employee), have read, understand, and agree to adhere to the City of Minneapolis AWA Policy, the AWA Procedures and the terms as described in this agreement. I will request any deviation from the approved AWA as soon as possible with my supervisor. I understand that either the City or I may terminate the Gradual Retirement Arrangement agreement with reasonable notice to the other party. (Note: Additional sheets may be attached to this agreement to document other important aspects of the AWA.)
