NICE guidelines (2OO8) and Dept of Health monitoring guidance (2010) have been integrated within this document Timescale of Service Level Agreement It is expected that the Community pharmacist and pharmacy staff will deliver smoking cessation support following the New Leaf operating guidelines to clients over 18 years of age registered with a Nottingham city GP until 2012. The terms of any extension may vary depending on resources, demand, capacity, performance, competition and the market. In the event of the contract expiring after a provider has begun the treatment Nottingham CityCare CIC would continue to fund those patients who had begun treatment. Background of CityCare NHS Nottingham Nottingham CityCare Partnership CIC has a registered population of more than 300,000 people. The population is diverse, multi-cultural and with high rates of deprivation and poor health.
