I, the Parent/Guardian of , give my permission and Agreement for my child to participate in any and all Krash activities for the 2017-­‐2018 season. Player commits to play Krash fastpitch softball and to attend all meetings, practices and tournaments as the team coach may designate. Player commits and agrees to all rules set forth by the coach and will always show good sportsmanship at all activities.
