THIS EMPLOYER AGREEMENT (the “Agreement”) is entered into by and between MC Medical LLC, a Georgia limited liability company (“Telehealth Provider”) and the other party set forth on the signature page (“Employer”) effective, when executed by both parties, upon the last date set forth on the signature page (the “Effective Date”). Telehealth Provider and Employer are referred to herein individually as a “party” and collectively as the “parties.”
