This Member Practice Agreement (this “Agreement”) is made by and between the undersigned Member (herein so called) and the Regents of the University of Colorado, a body corporate, on behalf of the University of Colorado, which includes the University of Colorado School of Dental Medicine, to be effective as of the date set forth opposite the signature of the Dean on the final page of this Agreement. The Dental Faculty Practice Plan (the “DFPP”) is limited to individuals who hold appointments of instructor or above as Faculty members of the University of Colorado, School of Dentistry.
