All Membership Agreements are subject to the acceptance and approval of OSR Physical Therapy d/b/a OSR Fitness Club (“OSR”). This Membership Agreement allows the member access to OSR’s exercise facilities during OSR’s regular hours of operation and in accordance with any OSR rules and regulations.
