have been received by me/us. I/We understand that the rules of the club may be changed from time to time and will be posted in the Club facility, and I/we agree to comply with all changes in such rules. I/We further understand that my/our membership may be suspended or terminated if I/we do not abide by the rules of the Club. I/We agree to pay a non-refundable $50 initiation fee and agree to pay for at least six months from this date the monthly dues as established by the Board of Directors of Dreams Landing Condominium. Dues will be billed monthly. After the initial six months, renewal will be automatic for additional periods of six month each unless I/we notify the Dreams Landing Property Management Company before the end of each six month period that I/we wish to terminate membership. My/Our membership will terminate at such time as I/we move from Dreams Landing Condominium. All Membership Agreements are subject to the acceptance and approval of OSR Physical Therapy d/b/a OSR Fitness Club (“OSR”). This Membership Agreement allows the member access to OSR’s exercise facilities during OSR’s regular hours of operation and in accordance with any OSR rules and regulations.
