Before completing this form, the employee requesting a telework arrangement must review Telework Policy 4.26. The nature of the work performed, the needs of the work unit, as well as the employee’s past work performance must be assessed by the supervisor and their supervisor to determine telework feasibility before entering into a telework agreement. Telework is neither an entitlement nor an organization-wide benefit.
