I, the undersigned, being the parent or legal guardian of the youth named above, do hereby consent to the participation of my youth in all the scheduled youth activities of CrossPoint Church, and any other supervised activities with the youth group, including youth events, camps, and trips. I give permission for my student(s) to be transported by youth leaders during special events. I agree to allow my student to be included in pictures, promotional material, and publications with and/or for CrossPoint Church.
