Any employee claiming this religious exemption must file a written request for exemption with CSEA. If the request is granted, the employee shall, as a condition of continued exemption from this requirement of paying service fees to CSEA, furnish CSEA with copies of receipts from the charity selected, as a proof that such payments have been made, or shall authorize payroll deduction of such payments.
