I, (Parent/Legal Guardian’s Name), would like to use the babysitting and child care services provided by certain personnel of the Royal Orchid Sheraton Hotel & Towers, and give my consent for my child(ren), (Child(ren)’s Name(s)), to be attended to by such Hotel personnel (the “Sitters”).
