This Professional Agreement (“Agreement”) is between Companion Benefit Alternatives, Inc. (“CBA”) and the undersigned, herein referred to as “Clinician.” The Clinician hereby expressly acknowledges his/her understanding that this Agreement constitutes a contract between the Clinician and CBA, a wholly owned subsidiary of Blue Cross and Blue Shield of South Carolina.
