CLINICAL STUDY AGREEMENT ZMLUVA O KLINICKOMSKÚŠANÍ This clinical study agreement (“Agreement”), concluded on 20-JUL-2022 and effective on the day following its disclosure in the Central Register of Contracts (the “Effective Date”), is entered into by and between Medpace Clinical Research, LLC, with its principal office and place of business at 5375 Medpace Way, Cincinnati, Ohio 45227 (“Medpace”), and Nemocnica s poliklinikou Brezno, n.o., Kardiologicka ambulancia, a clinical research site with its principal office and place of business at Banisko 273/1, 977 01 Brezno, Slovak Republik, ("Institution”).
