Contract Type FiledDecember 30th, 2021This Service Level Agreement (the “SLA”) is between Telechoice, LLC (hereinafter “Telechoice”) and its customers (the “Customer”) whoexecute a Service Activation Form, Statement of Work, online order, or an order of any kind (the “SAF”) for the purchase of certain services (the “Services”) as identified in the SAF; said SAF is incorporated herein by this reference. Telechoice and Customer may be referred to individually as “Party” or collectively as “Parties”.
