This BROKER-SHIPPER AGREEMENT (this “Agreement”), effective as of the date the last Party signs below (the “Effective Date”), is by and between Quick Logistics US, LLC, a Florida limited liability company (“BROKER”), and , with its principal place of business located at (“SHIPPER”). SHIPPER and BROKER may be referred to herein individually as a “Party,” and collectively as the “Parties”).
