Field Service Agreement Date: Customer Name: Address: Phone Number: Email: Make: Model: Serial Number: Scope of Repairs: Precision Heavy Equipment Repair Services LLC (PHERS) charges field service labor repair work at a rate of $149.00 per labor hour Portal to Portal (I.E. from the time that our truck leaves our facility until the time that it arrives back at our facility). PHERS also charges mileage at a rate of $2.50 per mile Portal to Portal (see description above).The undersigned agrees to pay these charges (Labor + Mileage as described above) plus the cost of all parts and supplies utilized on the job. All of these charges are payable on a COD basis unless a prior Credit Application has been approved and a credit line with PHERS has been established.
