PHERS also charges mileage at a rate of $2.50 per mile Portal to Portal (see description above).The undersigned agrees to pay these charges (Labor + Mileage as described above) plus the cost of all parts and supplies utilized on the job. All of these charges are payable on a COD basis unless a prior Credit Application has been approved and a credit line with PHERS has been established.
