This Agreement (“Agreement”) is made this first day of August 2021 (“Effective Date”) by and between MidHudson Regional Hospital of Westchester Medical Center, located at 1 Webster Avenue, Suite 400, Poughkeepsie, New York (“The Work Place”) and the City of Beacon, located at 1 Municipal Plaza, Beacon, New York (“Company”), to establish an Employee Assistance Program (“EAP”). The Work Place and Company may be referred to individual as a “Party” and together as the “Parties.”
