By signing below, you are agreeing to receive volleyball training from Little Rock Juniors Volleyball Club and our coaches. During this time of COVID-19 outbreak, you recognize that being in a group of individuals, such as at this training, represents a risk of transmission. While we are going to do what we can to prevent the transmission, we cannot guarantee that it will be successful. Therefore, in exchange for receiving volleyball training you agree to release Little Rock Juniors Volleyball Club and each of its individual instructors, agents, assigns and other member and parent institutions from liability for any injury or damage received through participation in this activity. This specifically includes any injury resulting from contracting COVID-19 or any other contagious disease or illness.
