This FY 2022 Annual Agreement (the “Agreement”) is made and entered into as of the date last signed by both parties below (“Effective Date”), by and between THE FORSYTH COUNTY DIVISION OF FAMILY AND CHILDREN SERVICES, a State of Georgia division of the Department of Human Services, having a principal office address of 6435 Shiloh Road, Suite C; Alpharetta, GA 30005, (hereinafter “Agency”) and FORSYTH COUNTY, GEORGIA, a political subdivision of the State of Georgia, acting by and through its governing authority, the Forsyth County Board of Commissioners (hereinafter “Forsyth County” or “County”), collectively referred to herein as the “Parties”.
