ELEMENTARY STUDENT ATHLETE PARTICIPATION AGREEMENT has been selected to represent the school on the Team. Please read and sign the following agreement with your child so that the expectations are well understood before the season begins. AGREEMENTAs a valued member of the Team, I recognize that I have two very different, yet important roles: 1. I am a student athlete.2. I am a representative of my school As an athlete: 1. I recognize that my role is based not only on my development as a player, but as a valued team member as well.2. My desire to improve my game is the most important aspect of my role as a student athlete, and I realize that this involves coming to practice regularly and maintaining my school work.3. I understand that any disciplinary action that is taken towards me for unbecoming behaviour will affect my place on this team. If I fall behind on homework and assignment completion, I may be removed from the team. This is a decision that will be done jointly by the coach
