Athlete Participation Agreement Form This document serves as an official agreement between the Brooke Point Women’s Basketball coaching staff and the potential participating athlete. Your signature on this document verifies your agreement to participate in the program and to adhere to the rules and expectations stipulated below for the 2018-2019 basketball season. Violation of any of these rules and expectations may result in dismissal from the program.

ELEMENTARY STUDENT ATHLETE PARTICIPATION AGREEMENT has been selected to represent the school on the Team. Please read and sign the following agreement with your child so that the expectations are well understood before the season begins. AGREEMENTAs a valued member of the Team, I recognize that I have two very different, yet important roles: 1. I am a student athlete.2. I am a representative of my school As an athlete: 1. I recognize that my role is based not only on my development as a player, but as a valued team member as well.2. My desire to improve my game is the most important aspect of my role as a student athlete, and I realize that this involves coming to practice regularly and maintaining my school work.3. I understand that any disciplinary action that is taken towards me for unbecoming behaviour will affect my place on this team. If I fall behind on homework and assignment completion, I may be removed from the team. This is a decision that will be done jointly by the coach
