Measuring the % of people accessing LTC programmes is challenging. However, Angus facilitates an increasing range of generic and disease specific programmes, e.g. Challenging your Condition, Expert Patient Programme on-line and Tayside Diabetes Education Programme. A range of support options have also been developed both on a generic and disease specific basis, e.g. Arthritis Forum, Angus LTC Support Groups, COPD and Cardiac Groups. Further developments planned for 2012 include chronic pain and fatigue management programmes. Consideration needs to be given with regards to collation of data to ensure an accurate% is reflected. The values will be reflective of the % of people who are newly diagnosed, accessing LTC programmes.This indicator is to be replaced for the 2012/13 review with '% of adults living in Angus with a long- term condition feeling supported to self manage
