This Risk Sharing Agreement (Agreement) is made and entered into by and between CareOregon, Inc. (CareOregon), Columbia Pacific CCO, LLC (CPCCO), and Tillamook County Community Health Centers (Provider) for the time period of January 1, 2021 through December 31, 2024. For purposes of this Agreement, CareOregon, CPCCO and Provider shall each be referred to individually as a “Party” and collectively as the “Parties”.
