THIS COVID-19 TESTING SERVICES AGREEMENT (“Agreement”) is entered into as of the last date of signature below ( “Effective Date”), by and between Covid Clinic, Inc., a Nonprofit 501c (3) located at 18800 Deleware St., STE 800, Huntington Beach, CA 92648 (“Covid Clinic”), and PALO ALTO UNIFIED SCHOOL DISTRICT, a public school district, located at 25 Churchill Ave., Palo Alto, CA 94306 (“School District”). Covid Clinic and School District are referred to herein individually as “Party” and collectively as “Parties.”
