This Peralta Community College District Student Health Program Agreement (the “AGREEMENT”) is entered into as of July 1, 2018, by and between Peralta Community College District (“PCCD”) and Roots Community Health Center, a California nonprofit corporation (“Roots”). PCCD and Roots may be individually referred to as “PARTY” or collectively as “PARTIES.”
