This Peralta Community College District Student Health Program Agreement (the “AGREEMENT”) effective as of August 5, 2022, is entered into by and between Peralta Community College District (“PCCD”) and Roots Community Health Center, a California nonprofit corporation (“ROOTS”). PCCD and ROOTS may be individually referred to as “PARTY” or collectively as “PARTIES.”
