This Application C Contract to participate in the SHSMD 25 Connections (“Event”) at the Hilton Anatole (“Event Facility”) over October 12 – October 15, 2025, including but not limited to move-in and move out dates (“Event Dates”) shall become effective when it has been submitted by the exhibiting company and accepted by the American Hospital Association for its operating unit, Society for Health Care Strategy C Market Development (collectively referred to herein for purposes of this Application C Contract as “SHSMD”). The individual signing this Application C Contract represents and warrants that he/she is duly authorized to execute this binding Application C Contract on behalf of the exhibiting company.
