This Executive Severance Agreement (this “Agreement”), made effective on <insert date> (the “Effective Date”), between Haemonetics Corporation, a Massachusetts corporation with its principal offices at 400 Wood Road, Braintree, Massachusetts, 02184, (herein referred to as the “Company”) and <Name> (the “Officer”).
