In consideration of the herein registered player, being my child/ward and being permitted to participate in an Organization of the Greater Cincinnati Youth Football League (GCYFL) and any related events or activities, the undersigned parent or guardian of the participant acknowledges, appreciates, and agrees to the following:

As a player of the Arizona Desert Scorpions you are expected to uphold the team’s policies and procedures at all times. You agree to participate in all team functions, practices, scrimmages, games, fundraisers, community service projects and pre-game meals.
