This Agreement (“Contract or Agreement”) is made and entered into as of the 29 day of May 2020, by and among the Office of the Connecticut State Comptroller, with offices located at 165 Capitol Avenue, Hartford, CT 06106, on behalf of the State of Connecticut, hereinafter referred to as “OSC”, “Comptroller” and/or “State”, and Happy Mountains, LLC (dba PhysicianOne Urgent Care) with its principal place of business at 31 Old Route 7, Brookfield, CT 06804, hereinafter referred to as “PhysicianOne ” and/or “Contractor”.
