This Actuarial Consulting Services Contract (Contract) is made by and between the Mississippi Department of Finance and Administration (DFA), and _________________________ (Consulting Actuary) conditions under which the Consulting Actuary agrees to provide services to the Mississippi State Agencies Self-Insured Workers’ Compensation Trust (Trust) and to DFA with regard to the Mississippi State Agencies Employment Compensation (Unemployment Self-Insurance) Revolving Fund (Fund) beginning May 10, 2019, subject to the following terms and conditions:

This Actuarial Consulting Services Contract (Contract) is made by and between the State of Mississippi State and School Employees’ Health Insurance Management Board (Board), acting administratively through the Department of Finance and Administration (DFA), and ________________, (Consulting Actuary) on October 1, 2018, under the following terms and conditions under which the Consulting Actuary agrees to provide services to the Board relating to the State and School Employees’ Life and Health Insurance Plan (Plan), and any other plans/programs for which the Board is or becomes responsible during the term of this Contract.
