This Agreement is made and entered into this day of , 2020 (hereinafter referred to as “effective date”) by and between , (hereinafter referred to as "Resident") and Adventist Health System/Sunbelt, Inc. d/b/a AdventHealth (hereinafter referred to as the "Hospital") on behalf of its , (hereinafter referred to as “Program” or “Residency Program”).
