This ACN Affinity Fundraising Program Agreement between ACN, Inc., a Michigan corporation with offices at 1000 Progress Place, Concord, North Carolina 28025 (“ACN”) and (the “Non-Profit”) with offices at is effective on the date of the second signature below (the “Effective Date”). As used herein, “Agreement” refers to the terms and conditions below, the ACN Affinity Fundraising Program Plan attached as Exhibit A (“Fundraising Plan”) and the ACN Affinity Fundraising Policies and Procedures attached as Exhibit B (“Policies and Procedures”).
