THIS AGREEMENT FOR TELEHEALTH SERVICES (the “Agreement”) is made and entered into this _ (the “Effective Date”) by and between Natchitoches Parish Hospital Service District No. 1 d/b/a Natchitoches Regional Medical Center (“NRMC”) and Natchitoches Parish School Board (“CLIENT”). (NRMC and CLIENT are each a “Party,” and collectively the “Parties”).
